Formerly known as Chronic Disease Management Plans (CDMP) / Enhanced Primary Care Plans (EPC)
To receive benefits from Medicare for Allied Health Services you will need to speak to your general practitioner (GP). They can talk to you about a Chronic Condition Management Plan.
They will let you know if you are eligible for the Medicare rebates. Eligible patients may access Medicare rebates for a maximum of 5 allied health services (total) in a calendar year.
Your GP will need to address this referral to Jointli and include the date of referral.
If you are eligible, they will allocate five (5) sessions to either Physiotherapy or Exercise Physiology. Here at Jointli we have different types of practitioners to assist with different conditions.
To ensure your Medicare sessions will be claimable with your practitioner, please check if they are a Physiotherapist or Exercise Physiologist on their profile on our About Us page. CLICK HERE FOR LINK
These Medicare sessions are only applicable to one-on-one sessions and cannot be used towards group therapy.
If for example you are going to start the Longevity program you typically may only need to see a team member for two (2) individual sessions. This means you will still have 3 x remaining sessions available to you.
